What does the first visit look like?
Your first visit takes place in your home whenever possible. Before we ever talk about your chart, we take the time to understand the real context of your life (your history, your family, and the life you're working to protect) and begin building a personalized, sustainable plan.
What if my needs change over time?
Continuity is a defining feature of the practice. If safety or health requires a transition into assisted living or skilled nursing, your care continues within that environment with the same physician and nurse practitioner team. You are never handed off to strangers.
Do you care for seasonal residents?
