Meet your care team.
Dr. Chapman and Emily Smith, APRN bring decades of combined experience in primary, functional, and geriatric medicine. They have worked alongside patients through independent living, care transitions, and complex health decisions, and they show up with the same calm, consistent presence every time.
The best healthcare happens within a relationship: one where your physician knows your history, your family, your goals, and the life you are working to protect.
Dr. Pamela Chapman, DO
Board-certified family physician. Expert in functional medicine. Board-certified in obesity medicine.
Dr. Pamela Chapman is a board-certified family medical physician, expert in functional medicine, and board-certified in obesity medicine. She is also a certified high-performance coach, bringing a comprehensive and highly personalized approach to modern primary care.
As the founder of a successful wellness and aesthetics practice and co-founder of The Bellanora Foundation, Dr. Chapman has built her career around one central belief: exceptional healthcare should be personal, proactive, and centered around the individual rather than the system.
She has had the privilege of serving as Medical Director for Village on the Isle and The Glenridge, two of the region's most respected communities. In these roles, she was entrusted with restoring continuity, strengthening communication between care teams, and guiding patients and families through complex medical transitions. This experience shaped her commitment to relationship-based care and reinforced the value of having a physician who truly knows you, especially during life's most important and vulnerable moments.
Dr. Chapman's approach combines clinical excellence with a deep understanding of the factors that influence long-term health, including lifestyle, environment, and personal goals. She is known for her calm, grounded presence and for creating a space where patients feel genuinely heard and cared for. Through unhurried, in-home visits, she develops individualized care plans designed to optimize health, preserve independence, and enhance quality of life.
Her passion for medicine was shaped early by her family's experience navigating serious illness, which instilled in her a lasting commitment to prevention, advocacy, and whole-person care. Today, that perspective guides how she partners with patients and their families, offering not only medical expertise but also clarity, consistency, and trusted guidance across every stage of care.
At the heart of her work is a dedication to helping patients live well now while thoughtfully planning for the future, supporting both the life they have built and the legacy they envision.
Emily Smith, APRN, FNP-C
Eight years in adult and geriatric medicine, dedicated to the unique complexities of aging.
Emily has spent the last eight years practicing adult and geriatric medicine. While nationally certified by the AANP in family medicine, her clinical focus is dedicated to the unique complexities of aging.
She partners with patients and communities across the continuum, from independent and assisted living to skilled nursing facilities, providing a vital sense of continuity for those navigating an increasingly complex healthcare system.
Emily believes that quality care is rooted in relationship. By serving as a steady, familiar face through both routine visits and new concerns, she establishes a partnership that allows her to truly empower those in her care. Through education and collaboration, she ensures her patients own their healthcare and understand they have a say in their treatment at every stage.
This thorough, tailored approach is evident in her work with complex medication management. By meticulously reviewing histories and removing unnecessary prescriptions, Emily helps her patients regain clarity and control over their health, ensuring that every treatment plan is as personalized as the person receiving it.
Emily earned her degrees from Ashland University and Mount Carmel College of Nursing in Ohio. She moved to Florida in 2021 with her husband and two children. Outside of her practice, she is an avid gardener and enjoys a good beach day.
